Explanation of Fees
The Initial Functional Physical is 75 minutes. The fee is $380, which includes in-office blood, saliva and urine testing and consultation time. The cost of recommended supplements and any additional outside lab testing are not included. Follow-up appointments for the Functional Physical are based on your unique plan. The cost of follow-up appointments vary based on tests performed and consultation time, billed in 15-minute increments.
In-Office, Phone, or Email Consultations
Your doctor at Lake Pointe will work with you to create a nutritional or detoxification program that is individualized for your needs. Follow-up phone and email consultations are an option to accommodate busy schedules or people who live outside of the Twin Cities metro area. An initial visit in person is required first. Consultation fees are $60 per 15-minute increment. Follow up consultations are scheduled as needed.
***Questions etc. directed to the doctor via telephone or email should not replace a scheduled consultation, and any correspondence that requires new instruction, case analysis, or prescription will be subject to a consultation charge. The fee is prorated according to the consultation time.
Acupuncture, Reiki and Energy Clearing are not covered by Insurance Plans
For Chiropractic patients without insurance, with out of network insurance, or those who desire to self-pay for services, Lake Pointe offers services for fees as listed below.
- Chiropractic Care: Initial Visit: $160.00
- Chiropractic Care: Follow Up Visit: $60.00
Chiropractic patients may decide to independently submit self-pay receipts to an out of network insurance plan. Lake Pointe will only provide diagnosis codes and CPT procedure codes upon request. Any additional documentation and/or paperwork is the patient’s responsibility.
The patient or responsible party must decide on the date of service whether to self-pay or submit a claim to their insurance plan. Once a chiropractic claim has been submitted to an insurance plan, it cannot be revoked on the basis of desiring to switch to self-pay.
Insurance only covers care to correct acute musculoskeletal injuries or complaints. Wellness or maintenance care is not covered by insurance. Fees for wellness or maintenance care or any other non-covered service is patient responsibility due at the time of service.
Information we receive from your insurance company about your plan and eligibility is not a guarantee of payment. Payment for services is a patient’s responsibility, and we highly recommend that the patient check with his or her insurance company before receiving services and be aware of any plan limitations.
Many insurance plans now include some form of an HRA, HSA, or FSA in conjunction with the benefit plan. While we will accept payment for services and certain products from such accounts, some insurance plans may deny payment for certain non-covered services and products per individual plan policy. Since each plan differs, we do not have a comprehensive knowledge of what services or products your plan may decline to pay. Please check your individual plan to ensure coverage before utilizing such funds for payment as any denied payment will become patient responsibility.
